A Clinical Observation of Paratesticular Tumors / 대한비뇨기과학회지

A clinical observation was made on 19 cases of paratesticular tumors in 19 patients which were neither resolved nor improved with primary medical and conservative treatment in the Department of Urology. Capital Armed Forces General Hospital during the period from March, 1977 to July, 1981. The following results were obtained. 1. The age was ranged from 20 to 45 years old and the most favorable age was in 3rd decade. 2. The main symptoms and signs were palpable mass in 73.1%. 3. Duration of symptoms from onset was over 1 year in 57.9% and under 1 year in 42.1%. The mean duration was 17 months. 4. Organ distribution was 5 cases in epididymis, 5 in spermatic cord and 9 in scrotal tunics. 5. 11 cases arose on the left side, 4 on the right and 4 on the midline. 6. There were benign tumors in 17 (89.5%) and malignant tumors only in 2 (10.5%) of all 19 cases. These malignant lesions were developed in the spermatic cord. 7. The lesions were managed by local resection in 16 cases, orchiectomy in 2 and epididymectomy in 1.

Venography in Varicocele / 대한비뇨기과학회지

In 20 patients of varicocele, both transscrotal and transfemoral phlebography to visualize the left renal, internal :spermatic and common iliac vein were performed during period from August, 1981 to March, 1982. The following results were obtained. 1. Among 14 pts. with passive reflux from the renal vein to the internal spermatic vein, proximal nutcracker phenomenon was observed in 10 patients, distal nutcracker phenomenon in 8 and both in 4. 2. Passive reflux to the internal spermatic vein without other abnormalities on phlebography was observed in 4 patients. Recurrent varicocele in 3 patient in this group who showed dilated duplicated internal spermatic veins were managed successfully by repeated high ligation. 3. In 2 patients showing renal-spermatic shunting vein and proximal nutcracker phenomenon concurrently, high ligation was performed distal to the point of the communication of shunting vein to the internal spermatic vein. 4. 8 patients with distal nutcracker phenomenon associated with passive reflux from the renal vein to the internal spermatic vein, I patient with distal nutcracker phenomenon and renal-spermatic shunt and 5 patients with normal venography were treated by ligation of internal spermatic vein, external spermatic vein and deferential vein through inguinal incision. 5. 1 patient with distal nutracker phenomenon without other abnormalities was managed by ligation of external spermatic and deferential vein at the level of internal inguinal ring. 6. Use of routine preoperative venography is essential to establish the etiology of the varicocele and its proper management.

Teratoma of the testis is a type of mixed tumor made up of various kinds of tissue representing the three primitive layers of blastoderm. Recently we experienced two cases of testicular teratoma in 23-year-old male patients, so we report these two cases of testicular teratoma with review of literatures.

Urachal anomalies are very rare. The umbilicus and the structures which lead to its formation rarely cause trouble except in patients with an error.
